No url, dude, just roll your own. Search on PDF, FDF, etc. Adobe's site should have info. PDFPlanet.com or some other site will have the info you need. Find an example .FDF file and you'll see how easy it is. The idea is to create a simple text file with .fdf extension in fdf format at runtime which contains form data (name, address, etc.).
